Key Responsibilities:

Conduct detailed annual home health risk assessments for Medicare Advantage members Review and evaluate prior and current medical histories, conditions, and medication lists Offer patient education on ongoing conditions and prescribed medications, empowering individuals toward better health management Identify care gaps and provide clear recommendations for addressing them Direct care responsibilities do not include prescribing medications or creating treatment plans

Qualifications:

Active Nurse Practitioner license in Missouri Current CPR certification (BLS) Board certification through AANP or ANCC required Masters degree in Nursing (MSN) from an accredited institution Minimum of 6 months recent Nurse Practitioner experience preferred Enjoy compensation of $80$90 per evaluation, plus reimbursement for mileage and allocated administrative time.
