We are currently recruiting for an RN or LPN to reliably serve patients in the following areas: Greene County MO + surrounding counties Flexible scheduling - Part Time Visits [15-45 Monthly RAI Visits] No Nights, No Weekends, No Holidays, No On-Call Mileage paid Pay Per Visit [PPV] What This Role Entails This role plays a vital part in supporting Medicaid clients receiving in-home services by conducting annual Resident Assessment Instrument (RAI) and Global Health Evaluation (GHE) assessments. These assessments help identify each client's strengths, needs, and overall health status to ensure they receive the appropriate level of care and services. Through observation, assessment, and collaboration, you will gather and document critical information used by the State of Missouri to determine continued eligibility and reauthorize in-home services. Your work helps ensure clients receive individualized care that supports their highest level of independence, health, and well-being.
