Youll be an integral part of a proactive approach to ensuring patients receive timely and comprehensive health risk assessments.

Schedule:

Flexible shifts available any day, Monday through Sunday Design your own schedule to fit your lifestyle Minimum hourly commitment per week or bi-weekly, ensuring adaptability

Key Responsibilities:

Complete annual home health risk assessments, typically 45 minutes per visit Assess and document patients prior and current medical history, conditions, and medication use Educate patients regarding their diagnoses and the use of their medications Identify care gaps and provide actionable recommendations for resolution No need to develop treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner license in Missouri Board Certification through AANP or

ANCC MSN
