Key Responsibilities:

Perform thorough home health risk assessments, averaging 45 minutes per visit Review and document medical, surgical, and medication histories Educate patients and caregivers about conditions, treatment options, and medications Identify gaps in healthcare delivery and provide actionable recommendations Manage administrative documentation related to each visit

Qualifications:

Active Nurse Practitioner license in Missouri Board Certification through AANP or ANCC Current BLS/CPR certification Minimum 6 months of recent experience as an NP preferred Master of Science in Nursing (MSN) from an accredited school
