Responsibilities:

Conduct comprehensive annual home health risk assessments for Medicare Advantage members within their residences Assess prior and current medical history, conditions, and review medication management Educate patients on their health conditions and prescribed medications Identify care gaps and offer informed recommendations to improve outcomes Perform timely, professional evaluations without creating treatment plans or prescribing medications

Qualifications:

Active Nurse Practitioner License in Missouri Board Certification through AANP or ANCC Current CPR certification (BLS) MSN from an accredited nursing program At least 6 months of current nurse practitioner experience preferred Benefit from highly competitive per-evaluation compensation, receiving $80$90 per completed assessment plus a mileage stipend and included administrative time.
