Key Responsibilities:
Conduct in-home annual health risk assessmentstypically 45 minutes eachon Medicare Advantage plan members Evaluate patients current and prior medical history, existing conditions, and medication regimens Educate patients about their health conditions and medications, enhancing their self-management Identify care gaps and offer recommendations to improve patient outcomes No responsibility for treatment planning or medication prescribing
Qualifications:
Active Nurse Practitioner license in Missouri Current CPR certification (BLS) Board Certification via AANP or ANCC Master of Science in Nursing (MSN) from an accredited institution At least 6 months of recent NP experience preferred In this per diem role, youll benefit from highly flexible scheduling, a supportive administrative team, and compensation of $80-$90 per completed evaluation, with additional stipends for mileage and administrative tasks.
