Key Responsibilities:

Perform annual health risk assessments for Medicare Advantage plan members in their homes Gather and review comprehensive medical histories, current health status, and medication usage Educate members about their medical conditions and prescribed medications Detect and report gaps in care, providing recommendations for follow-up No requirements for treatment planning or prescribing medications

Qualifications:
