Take on a dynamic, flexible per diem role as a Nurse Practitioner performing home health risk assessments for Medicare Advantage plan members across Cedar County, MO. Schedule:

Flexible shifts available Monday through Sunday Create your own schedule to accommodate personal and professional commitments Minimum weekly or bi-weekly hourly commitment

Primary Responsibilities:

Complete annual and post-discharge home health risk assessments for Medicare Advantage plan members Evaluate prior and current medical history, including conditions and medication use Educate members on their health conditions and medications Identify care gaps and recommend avenues for improved care coordination No requirement to create treatment plans or prescribe medications

Qualifications & Experience:

Nurse Practitioner License in the state of Missouri Board Certification through AANP or

ANCC MSN

degree from an accredited nursing program Current CPR (BLS) certification At least 6 months of recent experience as a Nurse Practitioner preferred
