Key Responsibilities:

Conduct home-based annual risk assessments for Medicare Advantage plan members Review and evaluate patients prior and current medical histories, including medication management Provide patient education regarding health conditions and prescribed medications Identify care gaps and suggest actionable recommendations for follow-up Each assessment averages around 45 minutes No responsibility for creating treatment plans or prescribing medication

Qualifications:

Nurse Practitioner licensure in Missouri Active CPR certification (BLS) Board certification (AANP or ANCC) Master of Science in Nursing (MSN) from an accredited institution Minimum 6 months experience as a nurse practitioner preferred As part of this role, you will receive $80$90 per assessment with additional compensation for administrative tasks and mileage.
