The ideal candidate will possess a strong background in patient care, with experience in various medical settings. This role requires excellent clinical skills, critical thinking, and the ability to work collaboratively with a multidisciplinary team to provide high-quality care to patients across different age groups and medical conditions. Responsibilities Conduct comprehensive patient assessments and develop individualized care plans. Administer medications and treatments as prescribed, ensuring adherence to safety protocols. Monitor patients' vital signs and report any changes in condition to the healthcare team. Provide education and support to patients and their families regarding health management and treatment options. Collaborate with physicians, specialists, and other healthcare professionals to coordinate patient care. Maintain accurate medical documentation in compliance with HIPAA regulations. Participate in quality improvement initiatives and adhere to NCQA standards. Assist with procedures such as IV insertion, catheterization, suturing, and other clinical tasks as needed. Provide emotional support and counseling for patients dealing with chronic illnesses or end-of-life care. Skills Candidates should ideally possess the following skills: Proficiency in EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks. Experience in critical care settings including ICU, Level I & II trauma centers, emergency medicine, and hospital medicine. Knowledge of acute pain management techniques and medication administration protocols. Familiarity with ICD coding (ICD-9 & ICD-10) and medical terminology. Competence in performing diagnostic evaluations, physical examinations, and specimen collection/processing. Ability to work effectively with diverse populations including geriatrics, pediatrics, and individuals with developmental disabilities. Experience in specialized areas such as hospice & palliative medicine, behavioral health, infection control, or home care. Strong interpersonal skills for effective communication with patients, families, and team members. Commitment to ongoing professional development through training in areas like pain management or health coaching.
