Job Description

LPN or RN Healthcare Home Case Manager - Chronic Illnesses RN Nurse Case Manager is responsible for managing high risk patients with chronic illnesses with provider, effective education, self-management support, and timely healthcare delivery to achieve optimal quality and financial outcomes. Nurse Case Manager performs collaborative with Providers to facilitate the best possible outcomes. Assist with Uniform Data Systems (UDS) and various quality improvement measures as directed.
