We are seeking an experienced Nurse Practitioner with a focus in Home Health to conduct comprehensive risk assessments for Medicare Advantage plan members in Adair County, MO, including communities such as Kirksville, Greentop, and Novinger. This per diem role provides significant flexibility, allowing you to create a schedule that fits with your lifestyle and other commitments.

Schedule:

Flexible shifts available Monday through Sunday Design your own work schedule Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

Perform annual home health risk assessments for Medicare Advantage members Evaluate and document medical histories, current conditions, and medication usage Educate patients and families regarding medical conditions and proper medication use Assess for gaps in care and recommend steps for resolution Visits may include annual health exams or post-discharge follow-ups No need for treatment planning or prescribing medications

Qualifications:

Valid Nurse Practitioner license in Missouri Board Certification (AANP or ANCC) Current CPR (BLS) certification Graduate of an accredited MSN program At least 6 months of recent Nurse Practitioner experience (preferred) Enjoy competitive earnings with $80$90 provided per evaluation, plus a mileage stipend and compensation for administrative activities.
