Key Responsibilities:

Conduct annual risk assessments in patients homes, typically within a 45-minute window per visit Evaluate patients prior and current medical history, current conditions, and medication usage Educate patients on chronic and acute medical conditions as well as prescribed medications Identify potential gaps in care and provide clear recommendations for follow-up Focus exclusively on assessment and education; the position does not involve implementing treatment plans or prescribing medications

Qualifications:
