Develop care plans including discharge planning, coordinate delivery among providers, and teach patients and families how to follow the plan. They may also help patients navigate insurance and other healthcare bureaucracies and access related social services. May work with patients who have chronic illnesses or other complex medical needs. Works at a hospital or other healthcare facility; may follow patients throughout the span of their treatment.

Responsibilities Manage and coordinate hospice care for an assigned caseload of patients. Perform comprehensive nursing assessments and develop individualized care plans. Collaborate with physicians, social workers, chaplains, aides, and other members of the interdisciplinary team. Provide education and emotional support to patients and their families. Maintain accurate documentation and ensure compliance with hospice regulations. Advocate for patients while promoting comfort, dignity, and quality of life. Qualifications Current Missouri Registered Nurse (RN) license. Hospice or home health experience preferred but not required. Excellent assessment, communication, and organizational skills. Compassionate, dependable, and committed to providing exceptional end-of-life care. Valid driver's license and reliable transportation.
