Key Responsibilities:

Conduct comprehensive home health risk assessments for Medicare Advantage members Review and document past and current medical history, ongoing medical conditions, and medication regimens Educate patients about their health conditions and prescribed medications Identify and note care gaps, offering recommendations to address these areas No need to prescribe medication or develop treatment plans

Qualifications:
