Schedule:

13-week contract, full-time Monday through Friday Daily visit quota; occasional on-call may be required

Responsibilities:

Deliver high-quality in-home nursing care and skilled interventions Complete patient assessments and timely OASIS documentation Collaborate closely with physicians and multidisciplinary care teams to create and deliver individualized care plans Provide wound care, medication management, patient education, and disease process monitoring Communicate effectively with team members regarding patient condition changes and updates

Qualifications:

Active RN license At least 1 year of recent home health RN experience OASIS documentation experience required Familiarity with HomeCare HomeBase (strongly preferred) Reliable transportation and valid drivers license Able to maintain a structured full-time schedule and manage multiple daily in-home visits Why Choose This Role?
