Responsibilities:
Perform comprehensive annual home health risk assessments for Medicare Advantage members Review and document prior/current medical history, existing medical conditions, and medication regimens Provide tailored education to patients about their medical conditions and prescriptions Identify care gaps and recommend next steps to close them

Qualifications:
Active Nurse Practitioner license in Missouri Up-to-date CPR certification (BLS) Board certified via AANP or ANCC At least 6 months of recent Nurse Practitioner experience preferred MSN degree from an accredited nursing school Benefit from competitive pay of $80-$90 per evaluation, in addition to a mileage stipend and compensation for administrative time.
