Responsibilities:

Conduct annual or post-discharge home health risk assessments in member residences Review and assess medical history, ongoing medical conditions, and current medications Educate patients about their conditions and prescribed medications Identify gaps in care and offer recommendations for improvement No development of treatment plans or prescribing required

Qualifications:

Active Nurse Practitioner license in Missouri Current BLS (CPR) certification Board Certified by AANP or

ANCC MSN
