Key Responsibilities:

Perform in-home annual and post-discharge health risk assessments for Medicare Advantage members Review patients medical history, conditions, and medication regimens Offer clear, empathetic education regarding health concerns and prescribed therapies Identify gaps in care and suggest evidence-based solutions to improve patient outcomes No requirement to develop treatment plans or prescribe medications

Qualifications:

Nurse Practitioner License valid in Missouri Current CPR certification (BLS) Board Certified through AANP or ANCC Completion of an MSN program from an accredited nursing school

Preferred:

At least 6 months of current NP experience You will have the opportunity to make meaningful connections with patients in West Plains, Mountain View, Willow Springs, and neighboring communities, with each risk assessment averaging 45 minutes. Compensation includes $80-$90 per evaluation, along with a mileage stipend and paid administrative time, recognizing your expertise and attention to detail.
