Treat wounds from clients recently discharged from the hospital Administer medications and treatments as prescribed by physicians. Conduct thorough health assessments and develop personalized care plans tailored to each client's needs. Provide wound care, manage dressings, and monitor healing to ensure client safety and well-being. Educate clients and their families on managing their health conditions, medications, and nutrition. Collaborate with a multidisciplinary team to deliver comprehensive care, ensuring all health needs are addressed. Monitor vital signs and report changes to the care team promptly. Assist clients with daily living activities, including mobility, hygiene, and nutrition. Conduct quality assurance visits and new client assessments to maintain high care standards. Supervise and train caregivers and nursing staff, fostering adherence to regulations and quality care protocols. Maintain accurate and up-to-date documentation of all client care activities and progress. Requirements Valid RN in the state of Missouri. Has at least 1 year of experience dealing with wound care. At least 2 years of nursing experience, with a minimum of 1 year in home care or a related field preferred. Strong clinical assessment skills and knowledge of home care practices; experience with geriatrics is a plus. Excellent communication and interpersonal skills to effectively build rapport with clients and their families. Ability to work both independently and collaboratively within a dynamic team environment. Proficient in electronic health records (EHR) and other relevant healthcare technology. CPR and First Aid certification required; CPR Instructor certification is a bonus. Knowledge of Missouri Home Health regulations is strongly preferred. Reliable transportation and a willingness to travel within the St Charles, MO areaPay:
