Youll serve Medicare Advantage plan members by completing thorough in-home health risk assessments within Jefferson County, working in areas such as Arnold, Imperial, Festus, and Barnhart. Each visit averages 45 minutes and may include annual health assessments or post-discharge evaluations.

Key Responsibilities:

Conduct home health risk assessments for Medicare Advantage members Review prior and current medical history, chronic conditions, and medication use Educate patients on health status and medication management Identify and document care gaps, offering recommendations for follow-up No treatment planning or prescribing required

Qualifications:

Active Nurse Practitioner license in Missouri CPR (BLS) certification Board Certified through AANP or

ANCC MSN
