Key Responsibilities:
Conduct home health risk assessments for Medicare Advantage members in various Franklin County communities Review and document patients medical history, underlying conditions, and medication usage Offer education related to diagnoses, prevention, and current medications Identify gaps in care and recommend appropriate follow-up or resources, without prescribing medications or creating treatment plans
Qualifications:
