Primarily Day Shift / Monday-Friday, with 1 week call rotation per month Bring Your Clinical Expertise to a Team That Values Quality Care We are seeking a knowledgeable, compassionate, and detail-oriented Registered Nurse (RN) Wound Care Nurse to join our clinical team at our skilled nursing facility. The RN Wound Care Nurse plays an important role in the prevention, assessment, treatment, and monitoring of wounds and skin conditions. This position works closely with nursing leadership, physicians, providers, dietary, therapy, CNAs, and other members of the interdisciplinary team to promote healing, prevent complications, and provide exceptional resident-centered care. Key Responsibilities Perform comprehensive skin and wound assessments and accurately document findings Monitor and evaluate pressure injuries, surgical wounds, diabetic wounds, vascular wounds, skin tears, and other alterations in skin integrity Perform wound treatments and dressing changes according to physician/provider orders and facility protocols Measure, photograph, stage, and document wounds as appropriate Monitor residents for changes in wound condition and promptly communicate concerns to providers and nursing leadership Develop and implement individualized wound prevention and treatment interventions in collaboration with the interdisciplinary team Participate in care plan development and updates related to wounds, skin integrity, nutrition, positioning, and pressure injury prevention Complete routine skin assessments and assist nursing staff in identifying residents at increased risk for skin breakdown Ensure appropriate pressure-relieving and preventative interventions are in place Collaborate with physicians, wound care providers, dietitians, therapy, nursing staff, and other clinical professionals Track wound progress and maintain accurate wound documentation and reports Educate nursing staff and CNAs regarding skin integrity, repositioning, pressure injury prevention, wound care procedures, and treatment protocols Provide resident and family education regarding wound care and prevention as appropriate Participate in wound rounds, clinical meetings, risk management, and Quality Assurance/Performance Improvement (QAPI) activities Assist with investigation and review of newly identified pressure injuries and other skin concerns Monitor wound care supplies and help ensure appropriate products and equipment are available Maintain compliance with facility policies and applicable state and federal requirements Assist nursing leadership with survey readiness related to wound care, skin integrity, and clinical documentation Serve as a clinical resource for nursing staff regarding wound prevention and management Qualifications Current, active Registered Nurse (RN) license in Missouri , or ability to practice in Missouri through applicable Nurse Licensure Compact privileges Graduate of an accredited nursing program Previous wound care experience strongly preferred Previous experience in skilled nursing, long-term care, rehabilitation, or post-acute care preferred Wound care certification preferred but not required Strong clinical assessment and critical-thinking skills Knowledge of pressure injury prevention, wound staging, treatment principles, and infection prevention practices Strong documentation and organizational skills Ability to effectively educate and support nursing staff Strong communication and interdisciplinary collaboration skills Commitment to resident safety, dignity, and quality of care The Ideal Candidate Our ideal Wound Care Nurse is proactive, clinically strong, organized, and passionate about prevention .
