Job Description

Advance your career as a Nurse Practitioner specializing in home health by performing essential health risk assessments for Medicare Advantage plan members within Franklin County, MO and the surrounding communities. This per diem opportunity offers flexible scheduling, allowing you to tailor your shifts throughout the week to suit your availability.
Schedule:
Flexible shifts available Monday through Sunday Set your own availability Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
Perform annual or post-discharge home health risk assessments in patient residences Evaluate and review medical history, current conditions, and medication regimens Educate patients on medical concerns, diagnoses, and medication adherence Identify gaps in care and collaborate on recommendations for follow-up or resolution No requirement for developing treatment plans or prescribing medications
Qualifications:
Active Nurse Practitioner License in Missouri Current BLS (CPR) certification Board Certification through AANP or ANCC At least 6 months of recent Nurse Practitioner experience preferred MSN degree from an accredited institution As part of this per diem role, you will earn competitive compensation of $80-$90 per evaluation, with an additional stipend for mileage and administrative time, reflecting the value of your time and expertise. Serving cities such as Union, Stanton, Berger, and other locations in Franklin County, you will play a vital role in improving patients health outcomes while maintaining control over your schedule.
