Job Description

This per diem Nurse Practitioner role focuses on conducting home health risk assessments for Medicare Advantage plan members throughout Franklin County, MO. Practitioners will visit residents in Washington and surrounding communities such as Union, Stanton, and Berger, helping individuals achieve better long-term health outcomes.
Schedule:
Flexible shifts available Monday through Sunday Choose your own schedule Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
Complete annual home health risk assessments for Medicare Advantage plan members in their homes Assess current and prior medical history, chronic conditions, and medication regimens Educate members regarding their diagnoses and prescribed medications Detect gaps in care and recommend actionable next steps to improve patient health No prescribing or treatment plan development requiredfocus is on assessment and recommendations
Qualifications:
Active Nurse Practitioner License in Missouri Current CPR certification (BLS required) Board Certification via AANP or ANCC Minimum six months of current NP experience preferred Master of Science in Nursing (MSN) from an accredited program Enjoy the autonomy to design your own work schedule while making a difference in peoples lives across Franklin County. Compensation for this position is competitive$80 to $90 per evaluationplus a mileage stipend and pay for administrative time.
