Key Responsibilities:

Complete annual home health risk assessments for Medicare Advantage members Review patients prior and current medical histories, conditions, and medication regimens Deliver education on disease processes and prescribed medications Identify gaps in care and recommend practical resolutions Visits generally last 45 minutes and may include annual assessments or post-discharge check-ins No requirement for prescribing medications or creating treatment plans

Qualifications:

Active Nurse Practitioner License in Missouri Master of Science in Nursing (MSN) from an accredited program Current Board Certification (AANP or ANCC) Current CPR certification (BLS) Minimum of six months experience as a nurse practitioner preferred
