Job Title MDS Care Plan Nurse Department Jones County Rest Home Full Time/PRN Full Time Job Summary The Minimum Data Set (MDS) Care Plan Nurse completes the Resident Assessment Instrument process in accordance with state and federal regulations and is responsible for the implementation, ongoing evaluation, and updating of each resident's comprehensive plan of care. The MDS Nurse also performs administrative duties as assigned. Minimum Qualifications Graduate of an accredited school of nursing with a current, unrestricted nursing license. RAC-CT certification or certification obtained within one year. Preferred Qualifications One year of experience in a long-term care environment or related field preferred. Previous experience completing MDS assessments preferred. Essential Duties & Responsibilities Monitor facility quality measures and assist in developing action plans for improvement. Assist in ensuring facility goals and objectives are achieved. Ensure MDS department compliance with local, state, and federal regulations. Ensure policies and procedures are followed regarding resident assessment, care planning, MDS documentation, and submission. Participate in and provide staff, resident, family, and visitor education. Actively participate in performance improvement initiatives, committee meetings, and audits. Assist with facility assessments and survey readiness activities. Work with administration to improve resident care outcomes. Address resident or care plan concerns and develop corrective actions. Schedule assessment reference dates to accurately capture reimbursement classifications. Ensure accurate and timely completion of all MDS assessments. Audit health records for supporting documentation and provide education as needed. Facilitate development and updating of resident-centered care plans with the interdisciplinary team. Prepare reports for surveys, admissions, transfers, and discharges. Perform other duties and responsibilities as assigned. Maintain competency in current standards of practice and professional development. Demonstrate excellent customer service consistent with organizational values. Support staffing needs by adjusting schedules or performing staff duties when needed. Physical Requirements Ability to sit, bend, stoop, kneel, crouch, perform overhead lifting, and perform common physical movements as required. Ability to lift and carry in excess of 50 pounds.
