Registered Nurse - Hospice Home care

Job Summary:

We are seeking a compassionate and experienced Registered Nurse (RN) to join our hospice and home care team in Montana. This is a 13 week interim contract with the possibility of extension. The RN will provide patient-centered care in home and community settings, focusing on comfort, dignity, and quality of life. This role requires strong clinical skills, emotional intelligence, and the ability to collaborate with an interdisciplinary care team to support both patients and families.

Key Responsibilities:

Deliver nursing care to hospice and home care patients in accordance with physician orders and care plans. Perform comprehensive patient assessments, monitor conditions, and adjust care plans as needed. Administer medications, treatments, and symptom management interventions. Provide education, emotional support, and guidance to patients and families regarding disease progression, care needs, and available resources. Collaborate with physicians, social workers, therapists, and other team members to ensure holistic care. Maintain accurate, timely, and compliant documentation of patient care. Ensure adherence to regulatory standards, infection control practices, and safety protocols.

Qualifications:

Current, active Registered Nurse (RN) license in Montana (or compact state license). Minimum of 1 year of nursing experience; prior hospice or home health experience strongly preferred. Strong clinical judgment, critical thinking, and problem-solving skills. Excellent communication and interpersonal abilities. Ability to work independently and manage time effectively while visiting patients in home settings. Valid driver's license and reliable transportation required.

Assignment Details:

Interim contract: 13 weeks, with potential for extension. Full-time hours with flexible scheduling based on patient needs. Temp to perm position
