Key Responsibilities:

Coordinate patient discharges and referrals efficiently, ensuring seamless communication between care teams and insurance providers Provide comprehensive case management by assessing patient needs and developing individualized care plans Collaborate with multidisciplinary teams to facilitate patient transitions and discharge procedures Maintain accurate documentation of patient interactions, insurance details, and referral activities in Cerner or similar electronic health record systems Engage with patients to educate and inform them about insurance coverage, discharge processes, and follow-up care instructions

Qualifications:

Valid RN license in Montana or ability to obtain prior to start BLS and CPR certification and state-specific licensure Minimum 1 year of care coordination or discharge planning experience; experience with Medicare and Medicaid processes Strong bedside manner, referral intake ability, willingness to commute to Billings, MT, and excellent communication skills
