Job Description
The primary purpose of this position is to serve as the Case Manager for managed care. This position will also serve as a RN health educator to promote, maintain, and improve individual and community health via educational processes. The goal of these activities is to create and deliver learning experiences designed to facilitate adaptations of behavior. Managed Care Case Management Identify clients to target for case management services through Care Impact/Virtual Health and referrals from various sources. Conduct a thorough assessment by review of claims history/prediagnosis/admissions history and various other reports in Virtual Health. Determine follow-up, monitoring frequency and assign case management status. Re-evaluate and update periodically according to client's needs. Provide education on disease process(es), importance of a medical home, medications and preventive health. Refer client to other supportive services as determined are needed. Document all pertinent information in CureMD. Compassionate and able to relate to different patients with various needs. Motivational to encourage patients to follow their care plans Critical thinking and problem solving to determine the best plan of care for each client based on assessment findings and review of data. Provide vaccines to patients at the Hampstead Annex. Serve as backup to General Clinic as needed in Burgaw. Work with the Communicable Disease staff during outbreaks to assist with case investigation, contact tracing, and community/provider/LTC facility collaboration. Assist with or take the lead role in mass immunization clinics. Enter immunizations given into NCIR. Participate in shelter rotation and recovery during hurricanes and other natural disasters. Employee will perform other duties as assigned by the Director of Nursing and Nursing Supervisor necessary to provide effective public health services. Duties may include participation in educational in-service programs, professional development and training exercises.

- Considerable knowledge of and skill in the application of nursing theory, practices, principles, and techniques employed in the field of public health and related programs; considerable knowledge of available resources and organizations and the ability to coordinate these as needed; general knowledge of current social and economic problems relating to public health.
- Ability to deal tactfully with others and exercise good judgment in appraising situations and making decisions, ability to secure the cooperation of patients, to maintain effective working relationships, and ability to record accurately services rendered and to interpret and explain records, reports and medical instructions.
- Proficient in MS Office and have the ability to learn to use web-based programs. Ability to use a variety of data dashboards to analyze and interpret relevant patient information.
- Graduation from a four-year college or university with a B.S. Degree in Nursing which includes a Public Health Nursing rotation and one year experience in Public Health Nursing; or graduation from an accredited school of professional nursing and two years of professional nursing experience one of which must have been in public health; or an equivalent combination of education and experience.
- Must have a valid NC driver's license.
- Considerable knowledge of and skill in the application of nursing theory, practices, principles, and techniques employed in the field of public health and related programs; considerable knowledge of and ability to apply the principles and practices of public health; considerable knowledge of available resources and organizations and the ability to coordinate these as needed; general knowledge of current social and economic problems relating to public health.
- Ability to plan, coordinate, and supervise the work of others; ability to deal tactfully with others and to exercise good judgment in appraising situations and making decisions; ability to secure the cooperation of clients, to elicit needed information, and to maintain effective working relationships; ability to record accurately services rendered and to interpret and explain records; reports, and medical instructions.
Required Minimum Training :
Graduation from a four-year college or university with a B.S. Degree in Nursing which includes a Public Health Nursing rotation and one year of Public Health Nursing experience.Additional Training/Experience :
Individuals, employed in a local health department, without a B.S. Degree which includes a Public Health Nursing Rotation will be required to complete a Public Health Nurse training program as a condition of continued employment.Individuals must also complete
ICS/NIMS
training.Equivalent Training and Experience :
Graduation from an accredited school of professional nursing and two years of professional nursing experience including one year in public health; or an equivalent combination of education and experience.- License or Certification Required by
Statute or Regulation :
