Liberty Health seeks an experienced MDS Coordinator to manage the Resident Assessment Instrument (RAI) and Minimum Data Set process within our Healthcare & Medical Services department. In this role, you'll ensure accurate, timely MDS completion, optimize reimbursement, and support regulatory compliance while collaborating with interdisciplinary teams to develop and monitor individualized care plans. You'll analyze quality measures, educate staff on MDS and documentation standards, and champion patient-centered care. Responsibilities Lead and coordinate the MDS process to ensure accurate, timely assessments Review clinical documentation and translate into appropriate MDS coding Collaborate with nursing, therapy, and social services to develop resident care plans Monitor quality measures and identify areas for clinical and compliance improvement Educate staff on MDS, PDPM, and documentation requirements Participate in interdisciplinary team meetings and care conferences Audit records for regulatory compliance and reimbursement optimization Submit MDS data per state, federal, and facility guidelines Required Skills MDS/RAI assessment and coordination PDPM reimbursement and case-mix management Clinical documentation and chart review Care planning in long-term and skilled nursing settings Knowledge of CMS and state long-term care regulations Quality measures and outcomes monitoring Interdisciplinary team collaboration Electronic health record (EHR) and MDS software proficiency Data analysis and report interpretation Staff education and training on MDS and documentation
