Job Description

Deliver expert nursing care in a field-based role as a Home Health Registered Nurse (RN). In this contract position, you will be a key contributor to clients health journeys, providing skilled clinical care and education to patients in their homes. As part of the interdisciplinary team, you will ensure a smooth continuum of care, optimize patient outcomes, and build trusting therapeutic relationships.
Schedule:
Monday Friday, 8:00 AM 5:00 PM Contract duration: 13 weeks Expected caseload: 2532 points per week (35 home visits daily) Travel within an assigned local territory (mileage reimbursement provided)
Responsibilities:
Conduct comprehensive OASIS assessments and design individualized care plans Deliver professional home health nursing: wound care, medication administration, IV therapy, and chronic disease management Monitor patient conditions, communicate changes promptly, and coordinate with other healthcare providers Educate patients and families on self-care, medication management, and disease process Maintain accurate, digital documentation in Home Care Home Base (HCHB) Participate in care conferences, team meetings, and continuing education as appropriate Ensure all care and documentation meet regulatory standards
Qualifications:
Active, unencumbered RN license in the state of practice At least one year of recent, hands-on home health RN experience Proficiency in Home Care Home Base (HCHB) and OASIS required Strong clinical assessment, communication, and organizational abilities Reliable transportation, valid drivers license, and current auto insurance necessary BSN preferred; knowledge of Medicare and home health compliance standards a plus Able to independently manage a flexible field-based workload Benefits include weekly pay, access to comprehensive medical, dental, and vision insurance, 401(k) options, dedicated support from expert staffing professionals, and mileage reimbursement.
