Non-Skilled Home Care Agency Job Summary:

We are seeking a compassionate and experienced Registered Nurse (RN) to join our non-skilled home care team. The RN will be responsible for conducting client assessments, developing individualized care plans, supervising caregivers/CNAs/Personal Care Aides, and ensuring all services are delivered in compliance with state regulations and agency standards. This role is essential in maintaining quality care and supporting clients in the comfort of their homes.

Key Responsibilities:

Conduct initial in-home client assessments and reassessments as required Develop and update individualized care plans based on client needs Supervise and provide clinical oversight to home care aides, caregivers, and other direct care staff Perform routine supervisory visits and document findings Communicate with clients, family members, physicians, case managers, and other healthcare professionals Ensure caregivers are following the established care plan and agency policies Provide education and guidance to caregivers and family members regarding client care needs Maintain accurate and timely documentation of assessments, visits, and care plans Assist with staff training, orientation, and competency evaluations Ensure compliance with state Medicaid and licensing requirements Report any changes in client condition, incidents, or concerns promptly

Qualifications:

Active Registered Nurse (RN) license in the state of North Carolina Minimum of 1-2 years nursing experience preferred Home care or community health experience preferred Strong assessment and documentation skills Excellent communication and leadership abilities Ability to work independently and manage multiple clients Reliable transportation and valid driver's license Current CPR certification preferred

Pay:

Competitive pay based on experience
