Your Responsibilities:

Provide direct, compassionate nursing care to patients in their homes. Assess patient conditions, develop and implement care plans, and monitor progress. Educate patients and families about managing illnesses and home care techniques. Coordinate and communicate effectively with physicians, case managers, and the broader healthcare team. Maintain accurate, timely documentation and adhere to all regulatory and agency standards.

Qualifications:

Active RN license in the state(s) required. At least one year of recent home health experience preferred. Strong assessment, clinical, and communication skills. Ability to work independently, with excellent organizational and time-management abilities. CPR certification.
