CVS Health is seeking an A1A Case Manager RN to coordinate care for patients with complex health needs. In this role, you will conduct clinical assessments, create and manage care plans, facilitate transitions of care, and collaborate with providers, members, and community resources to optimize quality, safety, and cost-effective outcomes. You will educate patients and families, monitor progress, and use data and clinical guidelines to reduce hospitalizations. Responsibilities Perform telephonic and/or virtual clinical assessments for members with complex needs Develop, implement, and update individualized care plans Coordinate services among providers, facilities, and community resources Facilitate safe transitions of care and reduce avoidable readmissions Educate members and caregivers on conditions, treatments, and self-management Monitor clinical outcomes and adjust interventions using evidence-based guidelines Document all activities accurately in electronic health records and case management systems Collaborate with internal CVS Health teams to support quality and cost goals Ensure regulatory and accreditation compliance in all case management activities Participate in continuous improvement, training, and performance initiatives Required Skills Care management Utilization review Clinical assessment Patient education Discharge planning Interdisciplinary care coordination Electronic health records (EHR) Healthcare quality metrics Motivational interviewing Regulatory compliance (CMS, URAC)
