We are seeking a dedicated Wound Care / Admissions Nurse (RN or LPN) to join our clinical team. Supportive and collaborative clinical team Strong leadership and hands-on training Competitive pay with Daily Pay option Comprehensive benefits Opportunities for professional growth and advancement Positive, team-oriented work environment Opportunity to build specialized clinical experience in wound care and admissions What You'll Do Wound Care Perform comprehensive wound and skin assessments upon admission and throughout the resident's stay Develop, implement, and monitor individualized wound care plans according to physician orders and facility protocols Provide wound treatments and preventive skin care Conduct regular rounds to monitor wound progression and overall skin integrity Identify changes in wound status and promptly communicate concerns to the appropriate clinical team members and providers Collaborate with nursing, dietary, therapy, physicians, and other members of the interdisciplinary team to support wound healing Monitor nutrition and hydration needs related to wound healing and communicate concerns as appropriate Maintain accurate wound assessments, treatment records, and clinical documentation Ensure wound care supplies and treatment areas are organized, clean, and properly stocked Follow infection prevention, safety, and wound care best practices Admissions Assist with the admission process for new residents Complete nursing assessments and obtain accurate clinical information upon admission Review hospital and medical records to identify diagnoses, medications, treatments, wounds, and other clinical needs Assess residents for skin integrity and identify existing or potential wounds upon admission Communicate admission findings and care needs to the interdisciplinary team Help ensure physician orders, medications, treatments, and care plans are accurately implemented Provide education and support to residents and families during the admission process Assist with coordinating a smooth transition from the hospital or other healthcare setting into the facility Ensure required admission documentation is completed accurately and timely What We're Looking For Current RN or LPN license in the state of practice Previous clinical nursing experience required Wound care experience strongly preferred Long-term care or skilled nursing experience preferred Experience with admissions is a plus Current CPR certification Strong clinical assessment and documentation skills Knowledge of wound prevention, treatment, and infection control practices Excellent communication and organizational skills Ability to prioritize multiple clinical responsibilities Compassionate, dependable, and resident-focused approach Ability to work independently while collaborating effectively with an interdisciplinary team Join the Cypress Valley Team If you're an experienced nurse who enjoys combining hands-on clinical care, wound management, and the admissions process , this is an opportunity to play an important role in improving outcomes and creating a positive experience for our residents from day one.
