Job Description
What We Offer Make a meaningful impact by connecting patients, providers, and communities to exceptional care as an RN Community Liaison with our Homecare Team . Novant Health is seeking a relationship-driven professional who is passionate about building trusted partnerships with physicians, healthcare facilities, and community organizations while expanding access to home health services. In this role, you'll serve as a key ambassador, educating referral sources, strengthening strategic relationships, and ensuring patients receive the right care at the right time. If you thrive on collaboration, customer service, and making a difference beyond the hospital walls, we'd love to meet you. What You'll Do Develop and maintain strong relationships with physicians, practice groups, hospitals, skilled nursing facilities, rehabilitation centers, assisted living communities, and other referral partners to promote home health services. Serve as the primary liaison for referral sources by responding promptly to inquiries, addressing concerns, and delivering exceptional customer service. Educate healthcare providers and community partners on home health programs, patient eligibility, referral processes, and available services to increase awareness and appropriate referrals. Collaborate with clinical and operational teams to support patient transitions of care, strengthen referral networks, and drive growth while ensuring an outstanding patient and customer experience.
