Job Description
Liberty Health seeks a Home Care Registered Nurse to deliver person-centered nursing care to clients in their homes. The RN completes comprehensive assessments, develops and updates care plans, administers medications and treatments, and monitors chronic and acute conditions. This role coordinates with physicians, behavioral health specialists, therapists, and social workers to ensure evidence-based, outcomes-focused care. The nurse educates patients and families, documents accurately in the EMR, promotes safety and infection control, and supports quality improvement initiatives while collaborating closely with public-sector partners.
