ROLE:
Bring comfort, confidence and compassionate care when patients and families need it most. AMOREM is seeking a registered nurse to provide skilled, responsive care to patients with serious illness and their families at night and on the weekends. In this role, you will assess patients' needs, manage pain and other symptoms, educate and support caregivers and coordinate care with physicians and the interdisciplinary team. You will help patients and families understand what to expect, make informed decisions and feel supported throughout the end-of-life journey. This position is ideal for a nurse who works well independently, responds calmly to changing patient needs, and values the opportunity to provide thoughtful, individualized care in patients' homes, community nursing centers and assisted living communities.
Requirements:
ESSENTIAL DUTIES & RESPONSIBILITIES
Respond promptly to patient, family, and team communications Assess patient and family needs and determine the appropriate clinical response Make patient visits as indicated and needed Provide nursing care in accordance with the individualized plan of care Assess and help manage pain, symptoms, medications, and changes in condition Educate patients and caregivers about the disease process, symptom management, medications, emergency responses, and what to anticipate Help patients and families make informed choices based on their goals and preferences Coordinate care with physicians and members of the interdisciplinary team Evaluate physical, psychological, social, spiritual, financial, and pre-bereavement needs Update the plan of care as the patient's condition or needs change Assess potential patients for hospice eligibility in collaboration with AMOREM providers and assist with admission when appropriate Connect individuals who are not eligible for hospice with other appropriate services and community resources Provide compassionate support at the time of death Assist in AMOREM patient care units when directed What You'll Bring Current, unrestricted North Carolina Registered Nurse license Two to three years of nursing experience Active patient contact or completion of a nursing refresher course within the past three years Current CPR certification Strong clinical assessment, communication, and decision-making skills Ability to work independently and collaborate effectively with an interdisciplinary team Flexibility to work various shifts Respect for the beliefs, backgrounds, values, and choices of every patient and family Valid driver's license, reliable transportation, and required automobile insurance The position requires the ability to travel throughout the assigned service area and perform the physical functions necessary to provide patient care, including standing, bending, kneeling, walking, and lifting approximately 30 pounds.
