Provide high-quality, compassionate care to elderly, ill, and disabled individuals in their homes. Conduct thorough assessments of clients' health conditions and create personalized care plans. 2.Whole Person Approach:
Embrace a holistic approach to care, considering the physical, emotional, and social well-being of each client. Collaborate with clients, families, and the care team to ensure a comprehensive understanding of individual needs. 3.Clear Communication:
Foster open and transparent communication with clients, families, and colleagues. Maintain detailed and accurate documentation of patient care activities, ensuring clear records for ongoing care coordination. 4.Safe Home Care:
Implement safety measures to create a secure and comfortable environment for clients. Educate clients and their families on health management and preventive measures. 5.Consistent Care Providers:
Work collaboratively with other care providers to maintain consistency in patient care. Support and guide caregivers to ensure they provide quality care in alignment with Helping Hands Care standards. 6.Client-Focused Approach:
Build meaningful relationships with clients and their families, fostering an atmosphere of trust and support. Strive to create an environment where clients feel treated "like family."Qualifications/Skills:
Valid Registered Nurse (RN) license in the state of North Dakota. Minimum of [X] years of nursing experience, preferably in home care or a related field.
