Job Description
Altru Rehabilitation Hospital, part of Altru Health System in Grand Forks, ND, is seeking a compassionate Case Manager to coordinate care for inpatient and outpatient rehab patients. In this role, you will manage a caseload from admission through discharge, conduct comprehensive assessments, develop individualized care plans, and collaborate with an interdisciplinary team to support functional recovery and independence. You will educate patients and families, arrange post-acute services, and connect patients to community resources.
