M-F 08:00-16:30, with weekend and call coverage (every 3rd weekend, 1-2 call shifts during the week Unit Notes
Home Health/Hospice office is located in the basement of the North Mandan Clinic;
Patient Population:
18+ with mostly geriatric patients
Setting:
patients are seen either in the home setting, Skilled Nursing Facilities, Assisted Living Facilities, or the hospital setting 2-3 Office meetings required per month
