Job Description

Position Summary The Registered Nurse (RN) Home Care Coordinator provides leadership, oversight, and coordination for the Public Health Unit's Certified Home Health Agency. This position is responsible for ensuring the delivery of high-quality, patient-centered home health services while maintaining compliance with federal, state, and accreditation standards. The Home Care Coordinator supervises clinical staff, oversees patient care operations, ensures regulatory compliance, participates in quality improvement initiatives, and collaborates with healthcare providers and community partners to meet the healthcare needs of residents throughout the service area. Essential Duties and Responsibilities Clinical Leadership Direct and oversee day-to-day operations of the certified home health program. Supervise, mentor, and evaluate nursing and home health staff. Ensure delivery of safe, effective, and evidence-based patient care. Participate in patient care planning and interdisciplinary team meetings. Assist with complex patient cases and clinical problem-solving. Regulatory Compliance Maintain compliance with Medicare Conditions of Participation, North Dakota regulations, and agency policies. Oversee OASIS documentation, care planning, and quality reporting requirements. Monitor clinical documentation for accuracy, timeliness, and reimbursement compliance. Assist with surveys, audits, and accreditation activities. Quality Improvement Lead quality assurance and performance improvement (QAPI) activities. Analyze quality metrics and implement corrective action plans as needed. Promote best practices and continuous improvement initiatives. Program Management Assist with budgeting, staffing, scheduling, and resource allocation. Monitor productivity and operational efficiency. Participate in strategic planning and program development. Support recruitment, on-boarding, and retention of clinical staff. Community Collaboration Maintain positive working relationships with hospitals, clinics, long-term care facilities, and community organizations. Represent the agency at community meetings and professional events. Promote home health services and public health programs throughout the region. Minimum Qualifications Current Registered Nurse (RN) license in North Dakota or ability to obtain license prior to employment. Bachelor of Science in Nursing (BSN) preferred. Minimum of 1 year nursing experience. Preference of one (1) year of supervisory or leadership experience. Home health, public health, case management, or community health experience preferred. Valid driver's license and ability to travel within the service area. Knowledge, Skills, and Abilities Strong knowledge of Medicare-certified home health regulations and standards. Understanding of OASIS documentation and home health reimbursement. Excellent leadership, communication, and organizational skills. Ability to manage multiple priorities and work independently. Strong critical thinking and clinical decision-making abilities. Proficiency with electronic health records and healthcare technology systems. Benefits Pay based on qualifications and experience.
