Job Description

Job Title:
Registered Nurse - Home & Facility Care (Variable Hours) Join our dedicated clinical team as a Registered Nurse providing compassionate, high-quality care in patients' homes, residences, or facility environments. This role focuses on direct clinical nursing services including IV therapy, venipuncture, advanced wound care and wound vac management, and hospice support. The RN will also serve as a key liaison, coordinating care plans and communication between physicians, facility staff, rehabilitation teams, patients, and families to promote optimal outcomes.
Schedule:
This position offers variable hours per week. The RN will participate in a rotating on-call schedule that includes weekday and weekend coverage. The ability to respond professionally and independently while on call is required.
Key Duties:
Assess patient condition, develop and implement nursing interventions, and document clinical findings accurately. Provide IV therapy and perform venipuncture safely and efficiently. Deliver wound care, manage wound vac therapy, and collaborate on wound healing strategies with the care team. Provide hospice and palliative care when indicated, offering support to patients and families. Maintain timely communication with MDs, facility staff, rehab providers and family members to ensure coordinated care.
Required Qualifications:
Valid Nebraska RN license and current BLS. Candidates must demonstrate strong computer literacy, excellent verbal and written communication, and customer service skills. The ideal candidate is team-oriented but able to work independently in community and facility settings. Prior nursing experience is required; home health and hospice experience are strongly preferred. A reliable personal vehicle, valid driver's license, and current car insurance are required for travel to patient locations. If you are a compassionate, skilled RN who enjoys autonomous practice and collaborative care coordination, we invite you to apply and become part of a supportive team focused on patient-centered outcomes.
