Key Responsibilities:

Perform annual home health risk assessments in members residences Assess prior and current medical history, overall health status, and medication usage Deliver clear, relevant education to members about their health conditions and medications Detect gaps in care and offer actionable recommendations to improve patient outcomes No responsibility for prescribing medications or generating treatment plans

Qualifications:

Active Nurse Practitioner license in New Hampshire Current BLS (CPR) certification Board certification through AANP or ANCC required At least 6 months of Nurse Practitioner experience preferred MSN from an accredited school of nursing
