Each visit, typically about 45 minutes in length, involves either an annual checkup or post-discharge evaluation, allowing for meaningful interactions and positive impact on individuals health journeys.

Key Responsibilities:

Perform annual or post-discharge home health risk assessments for Medicare Advantage plan members Assess prior/current medical history, chronic conditions, and medication usage Educate members about their medical conditions and pharmaceuticals Identify care gaps and make recommendations to improve health outcomes No treatment planning or prescribing responsibilities

Qualifications:

Active Nurse Practitioner license in New Hampshire MSN degree from an accredited nursing school Board Certification through AANP or ANCC Current BLS/CPR certification At least 6 months of recent NP experience preferred This opportunity allows you to build relationships with patients while working autonomously.
