A Registered Nurse provides medical care and treatment, educates patients about their conditions, and provides emotional support to patients and families. Works in hospitals, schools, clinics or community health centers. Works under the supervision of a physician or other specialist, and serves as the primary point of care for patients in a hospital setting. May specialize in emergency room work, or treatment for a particular condition, or specialize in working with infants, the elderly or other patient groups. May work as an advanced practice specialists and prescribe medications in addition to offering specialty care.

What You'll Do:

Provide skilled nursing care to patients in their homes Conduct comprehensive assessments and ongoing evaluations Develop and revise individualized care plans in collaboration with the care team Educate patients and families on disease process, medications, and care strategies Administer treatments and medications in accordance with physician orders Serve as Case Manager for your assigned patients, coordinating services and resources Communicate effectively with the interdisciplinary team to ensure continuity of care

Schedule Expectations:

Full-time | 5 days per week Shared on-call rotation

What You'll Need:

Active RN license in the State of New Hampshire Graduate of an accredited nursing program; BSN preferred 1-2 years of recent acute care or home health experience CPR certification Valid driver's license and reliable, insured vehicle Strong clinical skills, excellent communication, and a heart for community-based care Our mission is simple: deliver the highest quality care by hiring the best people.
