• Active Registered Nurse (RN) license in Vermont and New Hampshire or an unencumbered Nursing Compact License (RN Compact)
  
• Graduate of an accredited school of nursing
  
• Minimum 1 year of experience in Home Health and Hospice nursing (2 years preferred) — case management and care coordination experience preferred
  
• Current BLS; hospice/home health certifications a plus Responsibilities
• Develop individualized care plans upon admission in collaboration with physicians, patients, families, and caregivers — strong focus on home health care and hospice nursing
  
• Coordinate patient care through interdisciplinary communication, case conferences, and community resources (care coordination / case management)
  
• Perform ongoing clinical, functional, and psychosocial assessments to monitor progress and identify changes in condition
  
• Document evaluations, treatment goals, and outcomes accurately in the patient chart (home health documentation, EMR/OASIS familiarity preferred)
  
• Educate patients and caregivers on self-care techniques, medication management, diet, and physician-ordered instructions
