Full Job Description Job Summary We are a small long term care facility with 17 residents located in Windham, NH. We are looking for compassionate, friendly staff to help assist our residents. Great staff to resident ratios! We are looking for a PT RN/LPN, evening shift 3-8pm every OTHER Tuesday, Every Thursday and Every OTHER weekend. Responsibilities Perform thorough patient assessments, including vital signs, physical examinations, and medical history documentation. Administer medications and other specialized treatments following strict safety protocols. Manage complex care needs such as Alzheimer's disease, dementia, memory care, and behavioral health conditions with compassion and precision. Collaborate with multidisciplinary teams to develop and implement personalized care plans for patients across settings such as hospice care, or Physical Therapy. Maintain meticulous medical documentation ensuring compliance with HIPAA regulations. Experience Experience with geriatrics preferred. Prior involvement in specialized areas such as Alzheimer's/dementia/memory care or hospice enhances your ability to deliver compassionate support to vulnerable populations. Experience working with individuals with developmental disabilities or behavioral health challenges is preferred to ensure holistic patient-centered care. Knowledge of infection control practices, sterile techniques. Strong understanding of medical terminology, anatomy knowledge, and physiology principles to perform comprehensive patient assessments effectively.Benefits:
