Hour Job Summary:

We are seeking an experienced Registered Nurse (RN)

•Hospice to provide compassionate, patient-centered care to individuals and families receiving hospice services in a home-based environment. The RN will be responsible for comprehensive patient assessments, case management, care planning, medication management, patient/family education, and collaboration with the interdisciplinary team. The ideal candidate will have strong hospice nursing experience, excellent clinical judgment, communication skills, and the ability to work independently while traveling between patient locations.

Key Responsibilities:

Perform comprehensive assessments to determine patient appropriateness for hospice care. Assess patient and family needs and develop individualized plans of care. Monitor patient progress toward established goals and revise the plan of care as needed. Provide age-appropriate education and support to patients and families. Evaluate and document patient/family understanding of teaching and revise education plans as necessary. Manage an assigned hospice caseload and prioritize visits according to patient needs. Implement and evaluate evidence-based plans of care. Coordinate services and ensure appropriate utilization of hospice resources. Demonstrate knowledge of hospice regulations and insurance reimbursement guidelines. Maintain required visit productivity and scheduling standards. Complete admission assessments, transfers, revisit documentation, and discharge assessments within required timeframes. Complete electronic/laptop documentation and data transfers as required. Maintain accurate and timely patient records using the organization's EMR. Communicate patient progress and changes in condition to the interdisciplinary team (IDT). Attend and actively participate in interdisciplinary team meetings every two weeks. Perform Home Health Aide (HHA) supervision according to established standards. Demonstrate knowledge of pharmacology and safe medication management. Routinely reconcile medications and maintain current medication orders. Follow infection-control procedures and report infections appropriately. Maintain a safe environment for patients, families, and coworkers. Participate in Performance Improvement (PI) activities, quality reviews, and committee meetings as requested. Assist with orientation and mentorship of new employees. Cross-train and demonstrate willingness to learn and teach new skills. Attend monthly staff meetings and other required meetings. Accept additional assignments and responsibilities as requested. Support agency operations and contribute to quality patient care and service growth.

Required Qualifications & Experience:

2-3 years of RN experience in a home hospice environment Minimum 2 years of home hospice nursing experience required Experience working with an EMR Strong critical-thinking and clinical assessment skills Excellent verbal and written communication skills Ability to work independently and manage a patient caseload effectively Ability and willingness to travel/float to different counties based on patient and operational needs

Valid New Jersey Driver's License Education Required:

Associate's Degree in Nursing (ASN), OR Nursing Diploma with a valid

New Jersey RN License Required Licensure & Certifications:

Current, active

New Jersey RN License Valid New Jersey Driver's License BLS Certification Preferred Qualifications:

3+ years of RN experience Additional hospice or home-care nursing experience
